Client Caseworker, Temporary 4 months
(0.6 FTE, 22.5 hours per week) December 2024 to April 2025
St. Joseph’s Hospice is a charitable organization providing compassionate care and companionship to palliative clients, their caregivers and the bereaved, while recognizing their unique needs. Hospice programs and services are offered at no cost and are available irrespective of age, gender, religion, race, ethnicity or economic status.
Whether in the community, our wellness centre, or our 10-suite residence, St. Joseph’s Hospice provides specialized programs and services with a person-centred holistic approach. Individuals and families are intimately involved in creating their own plan of care which best addresses their unique needs. Our competent and committed Hospice staff and volunteers work together as a team, with other community partners and service providers, to provide full support and quality hospice palliative care.
Role Overview:
Reports to the Director, Community and Supportive Care and working within the Community & Supportive Care department, and in accordance with the Mission, Vision and Values of St. Joseph’s Hospice, the Client Caseworker is responsible for ensuring that Hospice clients and family members receive exemplary care and support within the community and through the Volunteer Visiting Program.
Key accountabilities of this position include, but are not limited to:
- Screening referrals for Hospice community support criteria
- Conducting home visits to perform initial holistic assessments of client/family units and risk assessment for volunteer support
- Assessing clients/family for psychosocial and spiritual needs and makes in-house referrals as appropriate
- Coordinate with partners and internal and external referral sources (i.e. St. Joseph’s Hospice Medical Director, Home and Community Care Support Services, Spiritual Care Practitioner, Grief and Bereavement Program Coordinator, Volunteer Coordinator) to prioritize admissions and ensure volunteer visiting matches take place effectively and in a timely manner.
- Communicating client/family needs to the Coordinator of Volunteer Services through referral for in-home volunteer visitors
- Liaising with referral partners regarding client/family needs and current status
- Monitoring volunteer reports and provides volunteers with direction, advice and support
- Reporting volunteer concerns to Coordinator of Volunteer Services
- Developing, documenting, and maintaining a client’s plan of care
- Ensuring regular assessment of client/family needs and adjusts plan of care accordingly
- Educating clients/families about additional community resources
- Advocating on behalf of client/families with SWLHIN PCOT care coordinators
- Completing client incident reports as necessary
- Inputting monthly client surveys into HPCO data base
- Maintaining current and accurate client data in data base and other formats
- Developing collaborative partnerships with organizations such as United Way and Community Support Services (CSS)
- In collaboration with members of the interdisciplinary team, providing input into the direction of policies and programs specific to community clients
- Representing St. Joseph’s Hospice in a positive and professional manner and promotes the best interest of the organization through speaking engagements and participation in supportive care programs such as memorial services and community dinners.
Qualifications:
To perform this role successfully, an individual must be able to perform essential duties satisfactorily. The requirements listed below are representative of the knowledge, skills, and /or abilities required.
Bachelor of Social Work (BSW) with current registration and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is preferred, however, applicants from allied health care professionals will be considered.
- 3 – 5 years of related experience including community service delivery
- Demonstrated knowledge of the palliative care-chronic disease continuum, as well as grief and bereavement
- Strong knowledge of related community resources
- Demonstrated knowledge and commitment to the principles of patient and family centered care
- Demonstrates a high level of communication and interpersonal skills to assess clients and build rapport with clients and family members
- Experience in community services program development
- Embodies a warm, caring, calm and genuine approach when interacting with others
- Experience working with volunteers
- Ability to maintain strict confidentiality, exercise tact and diplomacy, and exhibit good judgement and critical thinking
- A certificate in the Fundamentals of Hospice Palliative Care, or the commitment to obtain
- Demonstrated relationship management skills consistent with the Mission, Vision, and Values of St. Joseph’s Hospice
- Willingness to work flexible hours and possess reliable personal transportation and be approved by insurance carrier to drive personal vehicle
- Demonstrated knowledge and commitment to the Health Ethics Guide of the Catholic Health Alliance of Canada (CHAC)
